Supported Living Service (SLS) |

Independent Living Service (ILS) |
|
Supported Living Services are provided to adults between the ages of 18-59 with disabilities. The focus is on living independently in their own residences. The services/hours are authorized and funded by the local Tri-Counties Regional Center (TCRC) . Monthly service hours and training/supports are determined by the individual and the planning team using person centered planning. This assessment is then formalized into an Essential Life Plan for the Individual which helps the individual and the Regional Center follow how supports are working. New Heights supports the individuals by providing the support necessary so that the individual(s) can live independently in their own homes. |

Independent Living Service is a service for adults with disabilities whom might not need as much support services as an individual in Supported Living. Individuals whom receive services may live independently or with a roommate or spouse/family member whom does not receive services. Services support the individual to become or remain as independent as possible, thereby increasing self-sufficiency and decreasing dependency or paid support. ILS services service hours are funded by the Tri-Counties Regional Center (TCRC) and are determined by the individual and the planning team using person centered planning. |

Respite Care (RC) |

|
Respite Care is support service that provides intermitted relief for families caring for individuals with disabilities. All families at one time or another will experience the need for this type of relief from the stress of raising a child/family member with disabilities. New Heights Inc understands this need. We are vendored through the Tri-Counties Regional Center to provide this support to families with disabled individuals. Respite Care service hours/authorizations are funded by the Tri-Counties Regional Center (TCRC) and are determined by the individual and the planning team using person centered planning. |

Nursing visits are provided to individuals with disabilities through referrals, via SLS services programs from the Tri-Counties Regional Center (TCRC). New Heights Inc can provide a RN or an LVN to qualified (TCRC) aid recipients or individuals can request our nurse for private visits at a cost billed directly to them. All reports and information is kept private and confidential. TCRC recipient vendor rates are determined by TCRC, private nursing hours requested are determined at the going Medical rate for nursing. |

Aveana Gardening & Maintenance (LM) |
|
Aveana Gardening & Maintenance offers landscaping and maintenance work to residential and business owners. We mow, edge, sweep, haul away clippings and debris, weed and can handle any maintenance issues that you might want scheduled. Our rates are reasonable and we are very reliable. The jobs offered to us allow individuals with disabilities the opportunity to have steady employment within the community and earn a paycheck. Personal/Home Care Support (PHCS) |

New Heights Inc offers private in home care support to those whom cannot or do not receive aid or support from the Tri-Counties Regional Center. We offer this support to any age and any disability. Our personal home care support enables your loved ones and you to maintain a well-deserved quality of life. Individuals or elderly parents can live independently at home rather than be relocated to institutional settings or group homes. Our experienced, compassionate, and rigorously-screened/drug tested caregivers allow families seeking long term or even short term care for their loved ones or elderly parents a choice. They don't have to choose a nursing home if they don't want to. |
